How, where and when to apply
To find out if you can receive EI benefits, you must submit an application for EI on-line or in person at your Service Canada Centre , even if you received or will receive money when you become unemployed or if you do not have your record of employment (ROE).
You must however request your ROE from your last employer. If one or more ROE covering periods prior to your last employment are missing, you must still submit your claim for benefits and obtain the ROEs.
If getting your ROE is a problem, contact us at 1 800 206-7218. One of our agents will let you know how the ROE can be obtained or what is needed to calculate your claim.
If you are applying for maternity, parental, compassionate care or sickness benefits, you should apply as soon as you stop working.
Delaying in filing your claim for benefits beyond 4 weeks after your last day of work may cause loss of benefits.

What information/documents are needed to apply?

• your Social Insurance Number (SIN). If your SIN begins with a 9, you need to supply proof of your immigration status and work permit.
• a Record of Employment (ROE) from each job held over the last 52 weeks. Do not wait for receipt of your ROE to submit your application for benefits.  If you do not have your ROE after 14 days from the last day for which you were paid, contact us as soon as possible to know how to obtain it;
• personal identification such as your driver\'s licence, birth certificate or passport if you are applying in person;
• your complete bank information, as shown on your cheque or bank statement, or a voided personalized blank cheque from your current account. This will ensure that your payment of benefits will be made directly to your bank account with Direct deposit;
• a medical certificate indicating how long your incapacity is expected to last, if you are claiming sickness benefits;
• the SIN of the other parent if you are applying for parental benefits;
• a medical certificate for compassionate care benefits if you are applying for compassionate care benefits
• your detailed version of facts if you quit or were dismissed from any job in the last 52 weeks;
• details regarding your most recent employment: Your total salary before deductions including tips and commissions, your salary before deductions for your last week of work — from Sunday to your last day worked — gross amounts received or to be received: vacation pay, severance pay, pension, pay in lieu of notice or lay off and other monies.

需要什么什么资料和信息去申请？
1.您的工卡。如果您的工卡开头为9，你就需要有证明您的移民状态和工签许可。
2.员工工作小时记录，所有的员工记录在之前的52星期的。即使在等待您的员工记录的时候也要去提交您的EI申请。如果您没有收到员工记录（ROE）在距离您最后一天工作后的14天内，请尽快联系我们让我们来帮助您去获取员工记录。
3.个人证明，比如：驾照，生日证明或者护照（如果您亲自到办公室来申请）。
4.您的完整银行信息。能显示出您的支票和帐户其他信息，或者一张作废个人的支票在现在的帐户里的。这些可以保证在您收到EI给您寄来的钱时可以直接存入您的帐户里。
5.健康证明，可以表明如果您申请病假，这样我们预计在申请后病假福利后能够收到福利多久。
6.另一位家长的工卡号，如果您需要申请抚养费。
7.一个健康证明为救济金（这里指的是家里有重病患者需要照顾），如果您需要申请救济金的话。
8.工作详细证明，如果您在最近的52个星期里有推出或者因为违反规定被解雇。
9.一个关于您最近工作的详细信息，您的工资包括小费和提成，您在最后两个星期的工资--从星期日到你最后的工作日，你的总收入：假日工资，severance pay，退休金，pay in lieu of notice or lay off and other monies. （翻译的实在头疼了。想不出来了大家知道的请补充吧）。
